Adult Developmental Services

Adult Developmental Services offers three different levels of residential services:

Congregate: Congregate residential programs are located through out Albemarle County and City of Charlottesville. Referrals may come from Region Ten Intellectual Disabilities case managers locally and from other CSBs. Congregate programs offer continual support including overnight staff to respond to the health, medical and behavioral needs of consumers. Each program participant has a service plan developed to assist him or her to reach their goals. Our person-centered services strive to offer the greatest opportunities for unrestricted participation and community inclusion. Training may be provided in personal care, domestic skills, leisure time training, daily life skills and social skills. Assistance is offered in self-administration of medications.
Supported Living: In-home services provide intermittent supports to people living in their own home, apartment or with family. These services are designed to augment natural supports and are a less restrictive residential option. Services may take the form of monitoring medications, assisting with grocery shopping, health and medical appointment supports, etc.. The in-home model is designed to preserve independence while keeping people healthy and safe. Our person-centered services strive to offer the greatest opportunities for unrestricted participation and community inclusion. Training may be provided in personal care, domestic skills, leisure time training, daily life skills and social skills.
Sponsored Placement: Sponsored Placement provides services in a family setting. Consumers are matched with caring families who carry out instructional, planning and assistance services. For many people, sponsored placement services are ideal as they offer the nurturing of a family setting. These person-centered services strive to offer the greatest opportunities for unrestricted participation and community inclusion. Training may be provided in personal care, domestic skills, leisure time training, daily life skills and social skills. Assistance is offered in self-administration of medications.

Individuals Served: Individuals receiving residential support must be 18 years of age or older and have a documented diagnosis of intellectual disability. In accompaniment to intellectual disability, persons served may also have additional disabilities such as visual impairments, hearing impairments, mental illness, speech deficits, cerebral palsy, and/or other developmental disabilities. Residents must desire to live in the residential setting they are applying for and be able to be safely supervised and supported by the staff to consumer ratio of that residential setting.

Access to Program: Access to residential programs typically come from case management. Referrals accepted from local and state-wide ID case management.

Hours of Operation: Congregate and Sponsored placement services run 24/7; In Home services are delivered as scheduled on a person centered basis.

Payment for Services: Meadowcreek Center is accepts Virginia Department of Medical Assistance Services reimbursement through the ID/DS/SS Waivers. Private payors must pay full fee.

Day Activity Services - Meadowcreek Center

Meadowcreek is a center-based, community integration program that provides training, support and supervision to adults with intellectual disabilities. The program aims to help participants acquire, retain and/or improve their self-help, social and adaptive skills through person-centered planning, behavioral support and assistive technologies.

Individuals Served: Individuals attending the Meadowcreek Center must be 18 years of age or older and have a documented diagnosis of intellectual disability. In accompaniment to intellectual disability, persons served may also have additional disabilities such as visual impairments, hearing impairments, mental illness, speech deficits, autism, cerebral palsy, and/or other developmental disabilities. Participants must be able to be safely supervised and supported in a setting with a 1:7 staff to participant ratio.

Access to Program: Access to Meadowcreek Center typically come from case management. Referrals are accepted from local and state-wide ID case management.

Hours of Operation: Meadowcreek Center Hours of operation run from 8:30 am - 4:00p.m. with some schedule adjustments allowed for pre-authorized transportation.

Payment for Services: Meadowcreek Center is accepts Virginia Department of Medical Assistance Services reimbursement through the ID/DS/SS Waivers. Private payors must pay full fee.
