Services available:
The Crozet Clinic serves a wide variety of individuals who live in western Albemarle County. The following
services are provided:
Case Management - A supportive service that assists the individual in accessing the services that they
desire for their recovery.
Mental Health Support Services - Supportive services that help the individual with the challenges of daily
life in the community as they move toward recovery.
Crozet Psychosocial Rehabilitation – A day program that provides an environment where individuals can
develop or regain interpersonal and daily living skills, develop a supportive system of peers and providers
and optimize their ability to live successfully in the community.
Individuals Served - The program serves adults who are working towards their recovery from mental
illness.
Program Hours of Operation - Monday through Friday, 8:30 to 5:00

Payment for Services – Sliding scale and payment plan available. Funding sources: Medicaid and selfpay.
